You can apply online at Ontario Student Assistance Program. Paper applications are not available.

Who are my lenders and how can I contact them?

It is important to distinguish between your lenders and the provincial government division that receives and assesses your loan application. Keep in contact with your provincial government and all your lenders to keep them (and yourself) updated on information pertaining to your student loan.
