How do I apply?

  • You can apply online

  • If you cannot download and print an application online, please contact Northwest Territories Student Financial Assistance to request one by mail.

Where can I get a Form D (Student Enrollment Form) completed?

Undergraduate students:

If you are currently in Edmonton, you can take the Form D to the Student Access Centre, on the first floor of the administration building.

If you are not in Edmonton, you can mail it to the Registrar's Office (address below) along with a letter stating your name, student ID number and what you would like them to do (ie. fill out the Form D and mail it back to you). Make sure you sign the letter and it would be helpful to include a stamped, self-addressed envelope to reduce the possibility of having problems getting it back.

Graduate students:

If you are currently in Edmonton, you can take the Form D to the Faculty of Graduate Studies and Research (FGSR) office on the first floor of the administration building.

If you are not in Edmonton, you can mail it to the FGSR (address below) along with a letter stating your name, student ID number and what you would like them to do (ie. fill out the Form D and mail it back to you). Make sure you sign the letter and, to reduce the potential of experiencing problems receiving the form back, include a stamped, self-addressed envelope.
