Where can I get a Program Cost Form completed?
Undergraduate students:
If you are currently in Edmonton, you can take the Program Cost Form to the Student Access Centre, on the first floor of the administration building.
If you are not in Edmonton, you can mail it to the Registrar's Office (address below) along with a letter stating your name, student ID number and what you would like them to do (ie. fill out the Program Cost Form and mail it back to you). Make sure you sign the letter and it would be helpful to include a stamped, self-addressed envelope to reduce the possibility of having problems getting it back.
