Appendix 1 Parent(s) Financial Information:
If you are a dependent or 'Group A' student, this form outlining your parents' financial information is required and a parental contribution will be expected based on family size, income and allowable income deductions. Be sure to enter your application number on line 4.
Appendix 2 Spouse/Common Law Partner Information:
The information provided on this form determines the amount of assistance your partner is expected to provide for your education.
Appendix 3 School and Program Information:
This form is for students attending a private training school, the Justice Institute, Institute of Indigenous Government, or out-of-province school. This form provides the educational costs and program study dates for your school. It must be completed by the Registrar's Office at your school and must be included with your application. Contact the Registrar’s Office if you are out of province and need this form filled out.
Appendix 4 Students with Permanent Disabilities:
You must complete this form if you are a student with permanent disabilities who wishes to study at 40% of a full course load and is applying for the Canada Study Grant for Students with Permanent Disabilities. You may be eligible for grant funding of up to $5000 per program year.
Appendix 5 Transfer of School:
If you have already received assistance, you must submit this form to change institutions.
Appendix 6 Request for Review:
The request for review is to be used by students who wish to have their applications reviewed when exceptional circumstances exist that do not fall within standard program policy. Several policies are not eligible for review, such as maximum lifetime funding, weekly maximums, standard allowances, release of documents after End of Study date and grant eligibility. If you are an out-of-province student, you need to include a letter with the appropriate Appendix 6 form detailing the reasons for your request for review and all required documentation.
Appendix 7 Request for Reassessment:
This request is used to inform BCSAP of changes in your financial status or assets for you or your spouse, of a new source of income for you or your spouse, change in marital status, change in dependent information, change to program study dates, change of school BEFORE receiving assistance, change of living situation or change in previous post-secondary dates.
